The optimal scenario for a Café is to have an IBCLC present at every Café meeting.
In cases where there may be no IBCLC available to lead every Café, see guidelines below for Tiers 2 and 3 alternate staffing.
Tier 1 - IBCLC present at all Café meetings
Tier 2 - IBCLC present some Café meetings, other meetings may have Alternate Staffing (as defined below)
Tier 3 - Alternate Staff (a minimum of 2) present at every Café meeting
Following are links to some online breastfeeding training programs:
- *Tiers 2 and 3 are required to have IBCLC care available, free-of-charge, to any Café mom presenting with an issue requiring immediate intervention (mastitis, thrush, failure to thrive, weight loss, etc.)
- Outpatient lactation clinics using third-party insurance billing are acceptable. In areas where there is no clinical IBCLC availability, and only upon approval of Baby Café USA, a suggested donation by a private IBCLC referral may be asked of the mother.
Tier 1 - IBCLC present at all Café meetings
Tier 2 - IBCLC present some Café meetings, other meetings may have Alternate Staffing (as defined below)
Tier 3 - Alternate Staff (a minimum of 2) present at every Café meeting
- Alternate Staff may consist of the following:
- CLC, CBS, BCBC, CLE, Trained Breastfeeding Counselor (DPH, or Hospital/Clinic-based program)
- WIC Breastfeeding Peer Counselor, La Leche League Leader
- Midwife/OB/MD/RN
- In special cases, and on approval of BCUSA, alternate staffing adjustments can be made.
- *Please note: 2 Peer Counselors without certified training is not adequate staffing to handle emergent issues with babies/mothers unless there is an IBCLC/MD/Midwife on the premises
- Weekly verbal/text report given to an IBCLC by the alternate staff
- Quarterly face-to-face or conference call meeting between IBCLC and alternate staff to discuss case studies/issues

Baby Cafés are free, drop-in, informal breastfeeding support groups offering ongoing professional lactation care and intervention. Most are open for 2 hours at least once a week. Some communities have more than one Café, each holding meets on different days of the week.
